To initiate a payment for testing services, the following information will need to be provided:
1. Scheduled Test Date - this is the date the test is scheduled with UND Testing Services. This date was included in an email from UND Testing Services, sent to you at the time the test was scheduled.
2. Test Type -this is the test that was scheduled for you with UND Testing Services. This information was included in an email from UND Testing Services, sent to you at the time the test was scheduled.

Select the Test Type. If another drop down appears, select the appropriate test.
Price is dependent upon the selected Test Type.
For Scheduled Test Date, please enter the date that was scheduled with UND Testing Services. This date would be included in an email sent to you at the time the test was scheduled.
